This rewarding and varied role would suit someone who is highly motivated, has excellent customer engagement skills and enjoys working autonomously towards a target.    

The focus of the role is to identify, assess, on-board and manage a portfolio of community-based customers, which includes identifying their individual needs and requirements, conducting in-home clinical assessments, writing detailed case notes and developing tailored care solutions to enable our customers to enhance their quality of life. You will be inspired by making a positive difference to the families and achieving ultimate customer satisfaction.

Your strong account management and networking skills will enable you to build rewarding external relationships within your relevant communities and consumers both within your current portfolio and prospective. 

Your week will be spent mostly out on the road servicing customers in the region, and as required you will work out of our Port Macquarie office to liaise closely with our internal teams to ensure the smooth delivery of care services.

Skills and experience required:

  • Ideally previous case management experience and/or knowledge of the Community/Aged Care system and care packages
  • Exceptional customer service, negotiation and influencing skills
  • Experience in a results driven environment
  • Excellent ability to connect with multiple stakeholders and think commercially
  • Experience using a customer database or online CRM tool
  • A current valid driver's licence and reliable vehicle

To be considered for this role, you will have a qualification in health and/or community services with strong previous case management experience.
