We are seeking an experienced and passionate disability case manager to join our Disability Team at KinCare.

We require a candidate with NDIS knowledge and case management skills to facilitate services that enhance our customers’ quality of life and enable them to remain living at home.

Responsibilities include:

  • End-to-end case management of NDIS customers including conducting needs assessments and identifying person-centred solutions
  • Manage individual customer budgets to enable choice and empowerment
  • Work closely with the NSW team to ensure delivery of timely, quality services
  • Engage with customers in the community and liaise with external providers as required


  • Doing work that matters - making a real difference in the lives of vulnerable people
  • Challenge and variety working with multiple stakeholders and solving complex problems
  • Professional office environment and frequent community engagement 
  • Work with a supportive manager and team
  • Prime location in the upmarket Norwest Business Park - NEW Metro line now open
  • Active social club & health and wellbeing program including stretching break-out sessions and fresh fruit and snacks available daily

Skills and experience required:

  • Knowledge of the NDIS and transition process from State Government to NDIA funding 
  • Superior case management skills and previous experience in a similar role
  • Strong communication, customer service, negotiation and influencing skills
  • Excellent ability to connect and build rapport with multiple stakeholders
  • Resilience, with the ability to work under pressure and manage high volume workloads 
  • A reliable vehicle, current driver's licence and willingness to travel around Sydney
