The role:

This rewarding and varied role will focus on managing a portfolio of home care packages and other services for our community-based customers. A typical day in the community will consist of identifying customers' needs and requirements, conducting in-home assessments, writing case notes, promoting services and developing tailored care solutions to enable our customers to enhance their quality of life. You will be inspired by making a positive difference to families and achieving ultimate customer satisfaction.

This role gives you the flexibility to manage your week dividing your time on the road with time spent in our Brisbane office where you will be liaising closely with our internal teams to ensure the smooth delivery of services.

Skills and experience required:

  • Ideally a National Registration with Australian Health Practitioner Regulation Agency (AHPRA) 
  • Previous case management experience and knowledge of the Community/Aged Care system and care packages
  • Exceptional customer service, negotiation and influencing skills
  • Strong computer skills and articulate communication skills
  • Thrive on high attention to detail, strong resilience and working under pressure
  • A current valid driver's licence
